Documentation Services
Active Mindset Co. offers select documentation services for individuals who need clinically appropriate support for a workplace leave request or housing accommodation. Each request is reviewed individually and requires an evaluation. Completing an evaluation does not guarantee that documentation will be provided or accepted by the receiving organization.
Services may be available to established clients or individuals who complete the required clinical evaluation. Documentation is provided only when the clinician has enough information to support the request, the request falls within the clinician’s scope, and the receiving organization accepts documentation from that provider type.
FMLA Paperwork
We may complete mental health related FMLA certification forms when the requested information falls within the clinician’s professional scope. Documentation may address the need for continuous or intermittent leave, treatment frequency, and the anticipated impact of the mental health condition on the client’s ability to work.

Current clients may request paperwork when the clinician has enough treatment history to provide an informed clinical opinion. Individuals who are not established clients must complete a clinical evaluation and may be asked to provide supporting records. An evaluation does not guarantee that paperwork will be completed.

Employer or benefits administrator contact information
A description of the requested leave or workplace needs
Relevant records or information from other providers, if requested
Confirmation that the employer accepts certification from the clinician’s license type

Submit a documentation request and all available forms.
The practice reviews the request for scope and eligibility.
Complete a clinical evaluation or schedule a documentation appointment.
The clinician determines whether there is sufficient clinical support.
Approved paperwork is completed after payment and signed authorization.
The documentation is returned to the client or submitted as authorized.

FMLA evaluations and paperwork are self-pay services and are not billed to insurance. Fees must be paid before the completed documentation is released.

Allow 7–10 business days after the appointment, payment, and all required information have been received. Requests with missing information or additional employer questions may take longer.

The clinician cannot determine FMLA eligibility, approve leave, guarantee employer acceptance, or provide information that is not clinically supported. Requests may be declined when they fall outside the clinician’s scope, the available information is insufficient, or the employer does not accept certification from the provider’s license type. Fees cover the evaluation and professional time, not a guaranteed outcome.
Emotional Support Animal (ESA) Letters
Active Mindset Co. provides clinical evaluations for Emotional Support Animal documentation related to housing accommodations. When clinically appropriate, the therapist may provide a letter documenting the presence of a qualifying disability and the disability-related need for an assistance animal.

The service is available to established clients and individuals willing to complete the required evaluation process. New clients must complete at least two appointments before a letter can be considered. Eligibility is based on clinical findings.

Evaluations may be available to clients located in Texas, Virginia, or Washington, based on the evaluating therapist’s active license and current state requirements.

Submit an ESA service request and housing documentation.
Complete an initial clinical evaluation.
Complete any additional assessment or follow-up appointment requested.
The therapist reviews the clinical information and determines whether documentation is appropriate.
If approved, the letter is prepared after payment and signed authorization.

ESA evaluations and letters are self-pay and are not billed to insurance. Payment covers the evaluation and professional time, not a guaranteed letter.

An ESA letter is not a prescription, registration, or certification of the animal. It does not make an animal a trained service animal or grant access to restaurants, stores, workplaces, airplanes, or other public spaces. The practice cannot guarantee that a housing provider will approve the accommodation. A letter may be declined when the clinical findings do not support the request or the request falls outside the therapist’s scope.

Clinical evaluation is required
The therapist will use clinical judgment to determine whether the request is appropriate and supported by the available information.
Collaborate Openly
Please contact the practice at least 10 business days before your deadline. Incomplete forms or requests for additional information may require more time.
Submit requests early
The coordinator can assist with requests, scheduling, fees, and status updates. Questions about clinical decisions will be directed to the evaluating therapist.
